A proof of claim starts with the records

Official Form B 410 asks a creditor to identify a claim in a bankruptcy case. The official page includes the form and its instructions. Read those documents together with the court's notice and orders; they address different parts of the task.

Assemble a supporting file

• The debtor's exact legal name, court, and case number.
• The creditor's correct name and a reliable notice address.
• An account reconciliation showing principal, payments, credits, and any other claimed amounts.
• Contracts, invoices, judgments, or security documents supporting the claim.
• A note of disputes, prior filings, assignments, and documents requiring redaction.

Check the case-specific details

Ask counsel to confirm whether a claim must be filed, its classification, the deadline, and the permitted filing method. Keep a copy of what was submitted and its receipt. Completing an internal checklist does not establish that a claim is timely, valid, or allowed.
